Journal Club
About Journal Club
The SIREN Journal Club is a monthly, hour-long virtual session for discussing recent research in neurological, cardiac, respiratory, and hematologic emergencies. It meets on the 3rd Wednesday each month, with breaks in July, August, and December, and no registration required. Led by a multidisciplinary team, the journal club helps participants stay current on evidence-based treatments, diagnostic tools, and emerging findings. Attendees engage in critical review and collaborative discussion to support ongoing learning and clinical practice.
The objectives of this Journal Club are to:
- Critically evaluate recently published research findings in the fields of neurological, cardiac, respiratory, and hematologic emergencies to assess their applicability to clinical practice,
- Engage in collaborative discussions with peers and experts to enhance problem-solving skills and foster interdisciplinary learning; and
- Identify implications for practice/research.

All SIREN colleagues are welcome, including but not limited to:
Administrators, clinical psychologists, medical students, nurses, nurse practitioners, physician assistants, physician fellows, physician residents, physicians, retired health professionals, retired physicians, scientists, and social workers.

Dr. Benjamin Friedman, MD, presented: Minimum clinically important differences in acute pain: a patient-level re-analysis of randomized controlled analgesic trials submitted to the US Food and Drug Administration.
Dr. Friedman is a professor of Emergency Medicine and Vice Chair at Montefiore Medical Center in the Bronx, New York. His clinical focus is on evidence-based management of the broad range of problems that people have when they come to the emergency department. His research focus is on the management of acute pain include headache, low back pain, and appropriate use of opioids, and emergency medicine research methodology.
Dr. Akash Roy presented: Methodology for determining minimally clinically important differences in acute pain intensity with the double stopwatch technique.
Dr. Roy is an Assistant Professor of Biostatistics within the Department of Public Health Sciences at the Medical University of South Carolina (MUSC). Additionally, he is a member of the Data Coordination Unit, where he specializes in the comprehensive design of clinical trials, data analysis, and the implementation, maintenance, and management of systems for multicenter clinical trials.
His primary research interests include clinical trials, meta-analysis, survival analysis, and nonparametric rank-based methods, particularly in the context of clustered data. Currently, as a Co-Investigator of the SIREN Data Coordinating Center and the StrokeNet National Data Management Center, he is actively involved in study design and planning across both networks.
Drs. Elizabeth Munroe and Mark Tidswell presented: High-Flow or standard oxygen in acute hypoxemic respiratory failure.
Dr. Elizabeth Munroe, M.D., M.Sc., is an Assistant Professor in Pulmonary/Critical Care at Intermountain Medical Center in Salt Lake City. She completed medical school and residency at University of Chicago and fellowship at University of Michigan. Her research focuses on optimizing the use of fluids and vasopressors in early sepsis resuscitation. She is currently supported by a K23 career development award and is leading a pragmatic clinical trial of stroke volume-guided fluid resuscitation for patients with in-hospital sepsis.
Dr. Mark Tidswell is a Pulmonary, Critical Care, and Neurocritical Care physician and Professor of Medicine at UMass Chan Medical School-Baystate. He attended medical school at the Oregon Health Sciences University, completed internal medicine residency at Brown University, followed by research, pulmonary, and critical care fellowships at UC San Francisco and Stanford University.
He previously held faculty positions at UCSF, Brown University, and Tufts University School of Medicine. Since 1999, he has practiced critical care and neurocritical care, directed a critical care training program, and carried out clinical research at Baystate Medical Center in Springfield MA. He was a center co-PI in the NHLBI ARDS and PETAL networks. He leads clinical research trials in the Baystate Medical ICU to assess novel therapies for ARDS and sepsis. He is a multiple PI on the WINDSURFER UH3/UG3 grant to evaluate interventions for acute hypoxemic respiratory failure, and a co-PI on a Department of Defense grant to develop a new mode of mechanical ventilation for treatment and prevention of ventilator-associated lung injury.
Dr. Frank Guyette presented: Prehospital whole blood in traumatic hemorrhage — a randomized controlled trial.
Dr. Francis X Guyette MD, MS, MPH, FACEP, FAEMS is a Professor of Emergency Medicine at the University of Pittsburgh and has spent more than 35 years in Emergency Medicine and EMS. He is the Co-PI of the Linking Investigators in Trauma and Emergency Services (LITES) Network and the PAMPer (Prehospital Plasma), PPOWER (Phase II Prehospital WB), SWAT (WB in the trauma bay) and TOWAR (Phase III Prehospital WB) trials. His areas of expertise are in triage, the recognition of shock, hemorrhage, and prehospital resuscitation.
Dr. Jason McMullan presented: Comparison of inhalational methoxyflurane, intranasal fentanyl, and intravenous morphine for treatment of prehospital acute pain in Norway (PreMeFen): a randomised, non-inferiority, three-arm, phase 3 trial
Dr. Jason McMullan is an emergency medicine physician who completed his medical degree at the Medical University of South Carolina, followed by residency and an Emergency Medical Services fellowship at the University of Cincinnati. He is board certified in Emergency Medicine and EMS, and his clinical and research interests focus on prehospital and emergency care.
Dr. Ellen Sylvie Sanchez Mas presented: Long-Term Outcomes and Recovery Trajectories in Out-of-Hospital Cardiac Arrest A 2-Year Follow-Up of the Randomized Clinical TTM2 Trial.
Dr. Ellen Sylvie Sanchez Mas completed her medical degree at the Universidad de Costa Rica – Facultad de Medicina. While in Costa Rica, she was part of the Center for Research in Neurosciences at the University of Costa Rica, where she participated in both basic science and clinical research studies on Parkinson’s disease.
She completed her adult neurology residency at Baylor College of Medicine, where she participated in the Clinician Scientist Training Program and served as Education Chief Resident. In 2025, she was inducted into the Alpha Omega Alpha Honor Medical Society – Texas Chapter and received the Academy of Residents and Fellows Educators Excellence Award at Baylor College of Medicine.
She is currently completing her Neurocritical Care fellowship in the bicampus program at New York-Presbyterian Hospital at Columbia University and Weill Cornell Medical Center, where she is also part of the research group for the POST-ICECAP study.
Dr. Jonathan Beall presented: Adaptive increase in sample size when interim results are promising: A practical guide with examples
Jonathan Beall, PhD, is an Assistant Professor of Biostatistics in the Department of Public Health Sciences at the Medical University of South Carolina and a Senior Biostatistician with the Data Coordination Unit. He collaborates on the design, conduct, and analysis of multi-center randomized clinical trials within the SIREN and StrokeNet networks, including BOOST-3, HOBIT, CAPTIVA, RHAPSODY-II, and C3PO.
Dr. Beall’s research focuses on adaptive and Bayesian clinical trial design, Bayesian methodology, and latent variable methods, with the goal of improving the efficiency and rigor of complex clinical trials. He also serves on Data and Safety Monitoring Boards and contributes to national trial methodology efforts through professional service and peer review.
Dr. Byron Gajewski presented: Adding flexibility to clinical trial designs: An example-based guide to the practical use of adaptive designs
Byron Gajewski, PhD, is a professor of biostatistics & data science at the University of Kansas Medical Center in Kansas City, Kansas. He has been a member of the SIREN network through the HOBIT study since 2017. He is a fellow of the American Statistical Association and has a research interest in Bayesian adaptive clinical trial designs. He works with Jonathan Beall on the HOBIT trial as well as the STEP trial – stroke platform – led by Jordan Elm at MUSC. Today he will speak a bit on some experiences from a design from STEP that involves a group sequential design, and a concept called counter factual simulation that leads to a graphical representation of a promising zone.
Dr. Shelby Burks presented: The sodium-glutamate antagonist riluzole improves outcome after acute spinal cord injury: results from the RISCIS randomised controlled trial analysed using a global statistical analytic technique.
Dr. Burks is a neurosurgeon and physician–scientist whose work focuses on neurotrauma and peripheral nerve repair. Dr. Burks completed medical school with honors at the University of Miami and went on to neurosurgical residency and fellowship training at the University of Miami/Jackson Memorial Hospital.
His research career began with influential studies on traumatic brain injury biomarkers, and he later contributed to pioneering work on sciatic nerve injury and the first reported transplantation of Schwann cells into human peripheral nerve injury through The Miami Project to Cure Paralysis. He currently leads NIH- and AANS-funded research on repair strategies for large peripheral nerve injuries.
Dr. Ava Puccio presented: Early Blood Pressure Targets in Acute Spinal Cord Injury: A Randomized Clinical Trial.
Dr. Puccio is an associate professor of neurological surgery at the University of Pittsburgh and co-director of the Neurotrauma Clinical Trials Center. With over 25 years of experience in traumatic brain injury research, her work focuses on brain oxygenation, biomarkers, and improving outcomes in severe TBI.
Dr. Puccio has been a key leader in major national efforts, including the Phase 3 BOOST-3 trial, and has played an integral role in TRACK-TBI since its inception. She also directs the National TBI Biospecimens Repository and has led multiple collaborative biomarker initiatives.
A dedicated mentor, she received the 2025 Mary Ann Liebert Award for Science from the National Neurotrauma Society for her scientific contributions and leadership.

Dr. Guanbo Wang will be presenting: Using Effect Scores to Characterize Heterogeneity of Treatment Effects
Guanbo Wang earned his Ph.D. in Biostatistics from McGill University, during which he was also a trainee at the Department of Population Medicine at Harvard Medical School. He is a postdoc at CAUSALab in the Department of Epidemiology at the Harvard T.H. Chan School of Public Health. His research spans causal inference, data integration, and personalized medicine.
Dr. Valerie Durkalski-Mauldin will be presenting: The Predictive Approaches to Treatment effect Heterogeneity (PATH) Statement
Dr. Durkalski-Mauldin is a Distinguished University Professor in the Department of Public Health Sciences at the Medical University of South Carolina and serves as the Director of the Data Coordination Unit (DCU), a unit that specializes in the design, coordination and analysis of multicenter clinical trials. She serves as Principal Investigator of the SIREN DCC as well as a number of other NIH-funded statistical and data coordinating centers and collaborates on many large multicenter clinical trials in various therapeutic areas. These trials offer several methodological challenges that she addresses.
Dr. Jarron Mosier will be presenting “High-Flow Nasal Oxygen vs Noninvasive Ventilation in Patients With Acute Respiratory Failure. The RENOVATE Randomized Clinical Trial”.
Dr. Jarrod Mosier is an Associate Professor of Emergency Medicine and Internal Medicine at the University of Arizona. He is an Associate Program Director for the Pulmonary Critical Care fellowship and the medical director for the ECMO service. Dr. Mosier also serves as the national course director for The Difficult Airway Course: Critical Care. His research interests include advanced airway management, acute respiratory failure, ECMO, and critical care delivery.
Dr. Joseph Finney will be presenting “Midazolam and Ketamine for Convulsive Status Epilepticus in the Out-of-Hospital Setting”.
Joseph Finney, MD, is a Washington University pediatric emergency medicine and EMS physician. He is board certified in pediatrics, pediatric emergency medicine, and emergency medical services (EMS). He serves as the director of EMS at St. Louis Children’s Hospital and Missouri EMS for Children Director of Prehospital Readiness. Dr. Finney is committed to simplifying the prehospital care of children through interactive education, research, and quality improvement. In addition to his clinical and educational activities, Dr. Finney focuses his time on prehospital research in pediatric seizure management and manual ventilation with particular interest in the use of ketamine for prehospital benzodiazepine refractory status epilepticus.
Dr. Teresa May presented: Effect of level of sedation on outcomes in critically ill adult patients: a systematic review of clinical trials with meta-analysis and trial sequential analysis
Dr. Teresa May completed residency in Internal Medicine at Maine Medical Center, Pulmonary/Critical Care Fellowship at Maine Medical Center and Neurocritical Care Fellowship at Columbia Presbyterian/Cornell Hospital. She is now a pulmonary/medica/neurointensivist at Maine Medical Center in Portland Maine and Assistant Professor of Medicine at Tufts University. She was previously awarded a KL2 career development grant to investigate variation in sedation and neuromuscular blockade practices on outcomes after cardiac arrest and has current funding from NIGMS to evaluate rural disparities in cardiac arrest care. She is co-Investigator on the PREcision Care in Cardiac Arrest – ICECAP (PRECICECAP) study. (Let me know if you want more detail).
Dr. Michelle Nassal presented: A randomized trial of drug route in Out-of-Hospital cardiac arrest
Dr. Michelle Nassal completed an MD PhD at Case Western Reserve University then completed an Emergency Medicine Residency and Research Fellowship at The Ohio State University. She is currently an Assistant Professor in Emergency Medicine at OSU. She is also in her second year of an NHLBI K08 where she is evaluating end-tidal capnography for personalized resuscitation during resuscitation and the OHSU SIREN Network HUB Fellow and work closely with ICECAP trial enrollment at OSU.
